SHIPMENT ORDER INFORMATION
Flat Files are shipped directly
from the manufacturer to your location. The shipment is delivered by
truck to the curbside. Delivery drivers are not able to bring the
merchandise beyond the sidewalk. It is important to note the following
when ordering flat files:

We must have a contact
person and a daytime phone number to schedule the delivery time
and date. The freight company will notify you 24 hours before
delivery. |

|
Orders canceled more
than 24 hours after being placed will be subject to a 30%
restocking fee, plus freight charges if the Items are already in
transit. |

FLAT
FILE DELIVERY INSTRUCTIONS
Remember that this is a curbside delivery. The merchandise should
arrive in good order within 2 weeks. When shipment arrives, please
follow the guidelines listed below, before signing any paperwork.

Examine all
cartons closely on all six sides and verify number of cartons
received to quantity on freight bill and packing slip. |
| 2. |
If any carton is
damaged, do not refuse shipment. Open carton to
inspect the contents before signing. This is your right, if
damage is visible. |
| 3. |
You must write
description of visible damage on freight bill. Do not
leave this important function up to the driver. |
| 4. |
Please note the attached
SHOCK WATCH on carton. If red please comment
"inspection may be warranted" on Bill of Lading,
because there may be concealed damage. |

| Once
freight bill has been signed and driver is gone, thoroughly
inspect all merchandise for concealed damage. |

| 6. |
Save the cartons
if you find concealed damage. This is very important, since
without cartons concealed damage claims will be denied. |

7.
| A
request for inspection of concealed damage must be made within 4
days of delivery to the claims department of the Freight
Company. Report to the Freight Company-via telephone-any damage.
Have your delivery receipt available to provide necessary
information. Note the name of the person you spoke with,
date, and time of the conversation. |

are, Mon-Fri, 10:00 A.M. - 5:00 P.M., Pacific Time, once you
have spoken with the Freight Company regarding concealed damage.
We will then assist you in filing the claim with the Freight
Company, and help resolve any problems that have occurred. |
